Site: Medical-Acute Gastroenteritis (AGE) Log Review
Violation: On the AGE log, the last symptom column was not updated to reflect a food employee's actual time of last symptom. Medical staff stated that this information automatically populates in the AGE log for the electronic medical record system. The food employee reported to medical on March 19 at 1217 after an onset of symptoms at 1150. The AGE log listed the employee's last symptom as March 19 at 2100. However, a review of the employee's medical record showed the crew member's actual last symptom as March 21 at 1400. The crew member was isolated for the appropriate amount of time.
Recommendation: Ensure the AGE surveillance log entry for each passenger or crew member contain the following information in separate columns: (14) date of last symptom; (15) time of last symptom.

Site: Potable Water-Deck 7 - Far Point Record Review
Violation: The March 1 analyzer-chart record showed the chlorine residual level was above 5ppm from 1800 to 0245. A note on the report said there was a sensor failure between 1700 and 1800, and a manual sample was taken. However, no additional manual samples were recorded until 0245.
Recommendation: Ensure records from the halogen analyzer-chart recorder verifies the free residual halogen of between 0.2 mg/L (ppm) and 5.0 mg/L (ppm) in the water distribution system for at least 16 hours in each 24 hour period since the last inspection of the vessel. In the event of equipment failure, measure the free residual halogen by a manual test kit at the halogen analyzer at least every 4 hours.

Site: Galley-Deck 1 - Dishwash
Violation: A small crawling insect was on the ice deflector shield of the left ice machine. The ice machine was taken out of service to be cleaned and sanitized.
Recommendation: Protect food from contamination that may result from a physical, chemical, biological origin.

Site: Pantry-Deck 4 - Forward Housekeeping Pantry
Violation: Two slotted fasteners were on the ice thickness sensors in the ice machine. The fasteners were replaced with nonslotted fasteners.
Recommendation: Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Violation: The grouting where the two back countertops joined was recessed and in disrepair. This was corrected before the end of the inspection.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Bar-Deck 7 - Explorers Lounge
Violation: The countertop was chipped and in disrepair on the employee side of the bar.
Recommendation: Ensure multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams greater than 0.8 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices.

Site: Pantry-Deck 7 - Pool Grill
Violation: Food debris was on two previously cleaned pans in the clean storage racks. The crew took all the pans in the area to be recleaned and sanitized.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Violation: A black residue was on the gasket of Freezer #1. This was cleaned immediately.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Galley-Deck 1 - Chef's Table
Violation: The door to the crew women?s toilet room was not fully self-closing. The crew corrected this before the end of the inspection.
Recommendation: Ensure toilet rooms are completely enclosed and have tight-fitting, self-closing doors which are kept closed except during cleaning or maintenance.

Violation: A large amount of ice was on the bulkhead in the left corner closest to the entrance. This was immediately cleaned and sanitized.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Violation: The deck tile grouting was recessed and in disrepair.
