Site: Medical-Food/Beverage Questionnaire
Violation: During voyage 41120, a chef had an onset of acute gastroenteritis symptoms on 14 November at 07:30 and reported to medical at 07:57. On her food/beverage questionnaire she indicated that she went to the crew mess at 08:30 for breakfast that day. It was unclear if this was correct and if she broke medical isolation.
Recommendation: When food employees meet the case definition for AGE, ensure that they isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours.

Site: Galley-Deck 5 Pantry
Violation: In the walk-in dry storage room, one 5.5-pound can of artichokes was dented on the seam. The can was immediately discarded.
Recommendation: Ensure canned goods with dents on end or side seams are not used.

Violation: The juice machine was missing a fastener in the food-contact splash zone.
Recommendation: Ensure multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams greater than 0.8 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections.

Site: Galley-Deck 15 Solarium
Violation: The countertop mixer near the cold storage had two slotted fasteners in the food-contact splash zone above the bowl.
Recommendation: Ensure multiuse food-contact surfaces use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Site: Galley-Deck 16 Windjammer Hot Galley
Violation: Peeling silicone created a gap on the left side of the flat top range along the bulkhead.
Recommendation: Ensure equipment that is fixed because it is not easily movable is installed so that it is: (1) Spaced to allow access for cleaning along the sides, behind, under and above the equipment; (2) Spaced from adjoining equipment, bulkhead, and deckhead at a distance of not more than 0.8 millimeter or 1/32 inch; or (3) Sealed to adjoining equipment or bulkhead. Seal seams greater than 0.8 millimeters (1/32 inch), but less than 3 millimeters (1/8 inch), with an appropriate sealant or appropriate profile strips. Cover gaps greater than 3 millimeters (1/8 inch) with appropriate profile strips.

Site: Buffet-Deck 15 Windjammer Starboard Favorites
Violation: Three stacked pizza ovens had two slotted fasteners each on the inside bottom edge close to the oven door.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material. Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Galley-Deck 3 Warewashing
Violation: The final sanitizing rinse of the in-use rack-type conveyor dishwash machine had a weak spray pattern. This was corrected before the inspector left the area.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Violation: A large brown serving tray and empty used coffee pot were stored in the technical space.
Recommendation: Ensure soiled items to be cleaned in a warewashing machine are loaded into racks, trays, or baskets or onto conveyors in a position that (1) exposes the items to the unobstructed spray from all cycles; (2) allows the items to drain.

Violation: Six small, stacked plates out for passenger use were soiled with black debris.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Buffet-Deck 15 Windjammer Starboard Eggs and More
Violation: The outside of the top stacked bowl, ready for passenger use, was soiled with brown sugar. Staff repositioned the brown sugar to prevent soiling the previously cleaned bowls and removed the bowls for cleaning.
Recommendation: Store cleaned equipment and utensils, laundered linens, and single-service and single-use articles: (1) in a clean, dry location; (2) in a location where they are not exposed to splash, dust, or other contamination; (3) at least 150 millimeters (6 inches) above the deck unless in packages on pallets, skids, or racks, which may be 127 millimeters (5 inches).

Site: Pantry-Bionic Bar
Violation: At the handwash station inside the entrance of the pantry, the water temperature was 128°F as measured by the inspector. The temperature could not be adjusted by the user. The temperature was immediately lowered.
Recommendation: Ensure handwashing sinks are equipped to provide water at a temperature of at least 38°C (100°F) through a mixing valve or combination faucet. For handwash sinks with electronic sensors, and other types of handwash sinks where the user cannot make temperature adjustments, ensure the temperature provided to the user after the mixing valve does not exceed 49°C (120°F).

Site: Galley-Deck 3 Pastry
Violation: One handwash sink labeled 'Deck 3 FZ 8 Pastry' was out of service for 'water low/no pressure.' The work order was dated 31 October 2022. There were four additional working handwash stations in the pastry.
Recommendation: Keep handwashing facilities in good repair.

Site: Galley-Deck 8 Johnny Rockets Show Galley
Violation: Refuse was placed in a plastic, brown, rectangular container without a lid and stored on the bottom shelf of a front counter cabinet without a door. The area was in operation, but no crew members were observed discarding waste into this container.
Recommendation: Ensure receptacles and waste-handling containers for refuse and recyclables and for use with materials containing food residue are insect and rodent resistant and have tight-fitting lids.

Violation: Deck tile grout in front of the fryers was in disrepair.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Violation: Two small live flies and one live house fly were at the waiter station soiled area.
