Site: Medical-Deck A
Violation: During dry dock, a contractor's acute gastroenteritis (AGE) symptoms began at 0730 on 08 December 2022, but they did not report to medical until 1000 on 11 December 2022. The contractor experienced nine episodes of vomiting and one episode of diarrhea. The contractor self-isolated in their cabin after symptom onset, but their roommate continued to sleep in the room. A food/health questionnaire was not completed due to a language barrier.
Recommendation: Distribute questionnaires detailing activities and meal locations for the 72 hours before illness onset to all passengers and crew members who are AGE CASES. Ensure that the self-administered questionnaires contain all of the data elements that appear in the questionnaire found in Annex 13.2.2. Ensure that completed questionnaires are maintained with the AGE surveillance log. When any crew member (food or nonfood employee) meets the case definition for AGE, take the following actions for the asymptomatic cabin mates or immediate contacts of symptomatic crew: (1) restrict exposure to symptomatic crew member(s); (2) undergo a verbal interview with medical or supervisory staff, who will confirm their condition, provide facts and a written fact sheet about hygiene and handwashing, and instruct them to report immediately to medical if they develop illness symptoms; (3) complete a verbal interview daily with medical or supervisory staff until 48 hours after the ill crew members? symptoms began. Ensure that the first verbal interview is conducted within 8 hours from the time the ill crew member initially reported to the medical staff. If the asymptomatic immediate contact or cabin mate is at work, ensure that he or she is contacted by medical or supervisory staff as soon as possible. Document the date and time of verbal interviews. An immediate contact is someone sharing a cabin either full-time or part-time or sharing a cabin?s bathroom facilities. This includes boyfriends, girlfriends, spouses, and significant others. Ensure to document if the symptomatic crew member has or not cabin mate(s) or other immediate contact(s).

Site: Potable Water-Tank Maintenance Disinfection Record
Violation: Potable water tank 2C was painted with three layers of paint on 18 June 2021. The curing started at 1530 on 18 June 2021 and stopped at 1530 on 19 June 2021. The tank was disinfected to 50 ppm on 23 June 2021. The paint manufacturer's recommendations stated a 6 to 7 day cure time based on a tank temperature of 27C.
Recommendation: Ensure the interior coatings on potable water tanks are approved for potable water contact by a certification organization. Follow all manufacturers? recommendations for application, drying, and curing. For the tank coatings used, maintain the following onboard: (1) written documentation of approval from the certification organization (independent of the coating manufacturer); (2) manufacturers? recommendations for application, drying, and curing; and (3) written documentation that the manufacturers? recommendations have been followed for application, drying, and curing.

Site: Potable Water-Backflow Prevention Log
Violation: The backflow prevention log was not updated with the medical center's shower and backflow prevention device changes. The ship returned from dry dock on 15 December 2022, the day of the inspection.
Recommendation: Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing. Ensure a schedule for inspection frequency is set. Maintain a log documenting the inspection and maintenance in written or electronic form, and ensure it is available for review during inspections.

Violation: The backflow prevention device for the countertop juice machine was in disrepair with the vents blocked. Remediation began immediately.

Violation: The backflow prevention device for the countertop coffee maker was dripping water continuously and in disrepair. Disinfection protocols were discussed prior to installation of a new backflow prevention device.

Site: Recreational Water Facilities-Deck 9 Seaview Whirlpool Spas
Violation: A safety sign was missing at the entrance to the two whirlpool spas. The ship returned from dry dock on 15 December 2022 where they added ADA compliant ramps and replaced one of the whirlpool spas.
Recommendation: In addition to the safety sign requirements in section 6.7.1.1.1, install a sign at each whirlpool spa and spa pool entrance listing precautions and risks associated with the use of these facilities. Include, at a minimum, cautions against use by the following: (1) individuals who are immunocompromised; (2) individuals on medication or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure; and (3) pregnant women, elderly persons, and children. Additionally, caution against exceeding 15 minutes of exposure. Those under 16 years of age are considered children for the purpose of whirlpool safety sign requirements.

Violation: The front plastic protective shield for the ice machine evaporator unit was cracked in four places, which made this piece of equipment difficult to clean. Remediation began immediately.
Recommendation: Food contact equipment must be maintained in good repair and proper adjustment: (1) Equipment must be maintained in a state of repair and condition that meets the materials, design, construction, and operating specifications of these guidelines.

Violation: The inside of the ice machine's stainless-steel front cover had instruction labels on the inside area that were peeling and creating a difficult to clean surface.
Recommendation: Ensure nonfood-contact surfaces are constructed to allow easy cleaning and to facilitate maintenance.

Violation: The countertop milk dispenser had a glue-like residue on the outside door that made the surface difficult to clean. This area was clean at the time of the inspection.
Recommendation: Ensure nonfood-contact surfaces are designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: The dishwashing machine was not in service at the time of the inspection. Staff stated the ship had placed a work order and were currently waiting for parts.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Violation: The final rinse compartment manifold temperature for the dishwashing machine reached temperatures of 198F and 197F during the inspection. The inspector used a thermocouple measuring device with an accuracy of ( 0.36F). Remediation began immediately.
Recommendation: In a mechanical operation, ensure the temperature of the fresh hot water sanitizing rinse as it enters the manifold is not more than 90C (194F) or less than: (1) 74C (165F) for a stationary rack, single-temperature machine; (2) 82C (180F) for all other machines.

Violation: A toxic chemical in a spray bottle was stored on a countertop next to a coffee machine and coffee cups.
Recommendation: Store poisonous or toxic materials used in the cleaning and maintenance of food areas in a cleaning materials locker so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles.

Violation: The deckhead above the soft serve ice cream service area was perforated and had a gap between two tiles that exposed the plenum. Remediation began immediately. The area was not in service at the time of the inspection and no food was impacted.
Recommendation: Ensure that slots for supply ventilation over the void space (plenum) are not directly over food preparation, food storage, or clean equipment storage. This applies to vessels built or renovated in accordance with the 2011 Construction Guidelines or later.

Violation: The clear poly-glass acrylic finish on the bulkhead behind the soft serve ice cream maker was cracked and chipped which made the area difficult to clean.

Violation: The exhaust hood above the stove had collected condensate on the walls that dripped onto the deck below. No food was being cooked or impacted at the time of the observation.

Violation: The exhaust ventilation system was not working in an efficient manner which caused multiple areas to collect condensate. The exhaust hood above the soup station had condensate that dripped onto the deck below. No food was impacted during the inspection.
Recommendation: Ensure ventilation hood systems and devices operate effectively to prevent grease and condensate from collecting on the bulkheads and deckheads and remove contaminants generated by equipment located under them.

Violation: The couch cushions, decorative trash/paint can, and stage area had more than one days accumulation of dust. The area had been previously cleaned and was not in operation.
Recommendation: Ensure surfaces that children touch with their hands are cleaned and disinfected at least daily with products labeled by the manufacturer for that purpose.
