Item No.: 02
Site: Medical-Acute Gastroenteritis (AGE) Log
Violation: An entry on the AGE log for voyage #56890 (13 - 20 January 2023) indicated a food worker had AGE symptom onset on 14 January but did not report to medical until 17 January. Further review of the medical records showed that the report to medical entry on the AGE log was incorrect and was supposed to reflect 14 January. This was confirmed by reviewing the intake/triage log and the crew member's time sheet.
Recommendation: Ensure the AGE log contains correct information and matches the records obtained by medical staff.

Site: Galley-Deck 3 - Pastry
Violation: A crew member working bare handed at the dough sheeter had ink writing on the palm of his hand.
Recommendation: Ensure food employees keep their hands and exposed portions of their arms clean. Ensure food employees clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and: (5) During food preparation, as often as necessary to remove soil and contamination and to prevent cross- contamination when changing tasks.

Site: Provisions-Frozen Vegetables
Violation: Frozen pomegranate seeds leaked through the box and onto the box below.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination.

Site: Other-Bridge - Coffee Station
Violation: A slotted fastener was installed on the coffee press plate to the counter-mounted espresso machine. Old coffee residue was in the recessed area of the fastener.
Recommendation: Ensure multiuse food-contact surfaces use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Site: Galley-Qsine
Violation: Clear tape was on the front underside of the flat and grooved grill, making cleaning difficult.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Site: Galley-Sushi on Five
Violation: On the bottom of the fire shutter door, the gasket was torn in a few areas, which made it difficult to clean. Also, a large hole was in the center of the gasket.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Galley-Deck 3 - Dishwash
Violation: The final sanitizing rinse was not producing an effective spray pattern. It appeared the water pressure was low, which resulted in a bulb-like pattern to a narrow stream from each nozzle. The technician adjusted the spray arm which corrected the issue.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Site: Other-Bridge - Coffee Station
Violation: Old coffee residue of more than a day's accumulation soiled the coffee press plate to the counter-mounted espresso machine.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Galley-Tuscan Grille
Violation: Grease and dust were on the inside bottom surface of the previously cleaned bulkhead-mounted oven. The area was not in operation during the inspection.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Galley-Deck 3 - Potwash
Violation: The bucket fill faucet had a slow water leak.
Recommendation: Ensure a plumbing system in a food area is maintained in good repair.

Site: Galley-Dishwash
Violation: Above the soiled end of the rack-type dishwash machine, condensate was dripping from the inside of the hood overhang onto the machine. No clean items were impacted.
Recommendation: Ensure ventilation hood systems and devices operate effectively to prevent grease and condensate from collecting on the bulkheads and deckheads and remove contaminants generated by equipment located under them.

Site: Galley-Deck 3
Violation: Many food equipment items had been out of operation for months to more than one year. For example, in the pastry: (1) the deck-mounted mixer was out of service since 12 December 2022; (2) the adjacent cooking top was out of service since 5 April 2021; (3) the deck-mounted oven was out of service since 30 October 2022. In the soup station: (1) the chopper was out of service since 12 December 2021; (2) two large soup kettles were out of service since 5 April 2021 and 12 December 2021, and one small soup kettle was out of service since 2 October 2021. In the potwash, the left trolley wash was out of service since 12 December 2021. Work orders were placed for all equipment repairs/replacements. Some equipment replacement had been received onboard, but was incorrect and sent back to the supplier.
Recommendation: Ensure only articles necessary for the food service operation are stored in food preparation, food storage, and warewashing areas.

Site: Pantry-Pool Bar
Violation: One live fly was near the cleaning locker. The area was in operation and drinks were being prepared.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.
*Inspections scores of 85 or lower are NOT satisfactory
Vessel Sanitation Program
