Site: Buffet-Deck 11 - Beverage Station Starboard Side
Violation: Water continuously leaked from the atmospheric vents of the backflow prevention device for the soda dispenser inside the technical space below the unit. Water dripped into the deck drain.
Recommendation: Maintain backflow prevention devices in good repair.

Violation: Water continuously leaked from the atmospheric vents of the backflow prevention device for the coffee machine inside the technical space below the unit. Cleaning started immediately.

Site: Potable Water-Pipe Disinfection Log
Violation: The record for a pipe disinfected on August 27th stated the pipe was disinfected using 50ppm of chlorine from 0550 to 0902. Staff showed the inspector a plumbing system work permit that stated the pipe was disinfected until 1002. Staff reported that this was a recording error.
Recommendation: Maintain documentation of all inspections, maintenance, cleaning, and disinfection for 12 months and ensure it is available for review during inspections. Ensure records include a method of disinfection, concentration and contact time of the disinfectant, and a halogen value of less than or equal to 5 ppm before the tank is put back into service.

Site: Provisions-Deck 1 - Ice Box D01-7-008
Violation: A large icicle hanging from the deckhead hatch was in contact with a box of steaks stored below. The boxes were removed, and cleaning started immediately.
Recommendation: Do not store foods: (7) Under leaking water lines, including leaking automatic fire sprinkler heads, or under lines on which water has condensed; (9) Under other sources of contamination from nonfood items such as ice blocks, ice carvings and flowers.

Site: Pantry-Deck 19 - Forward Starboard Ice Machine
Violation: The top right bolt on the outside of the right evaporator panel was missing, exposing a threaded screw. This was replaced immediately.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: Water dripped from the deckhead onto the countertop in front of the coffee machine. No food was impacted.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: Two small flies were around the cold cuts station.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.
