Site: Potable Water-
Violation: THERE WAS NO DOCUMENTATION OF THE COATINGS USED AND THE RECOMMENDATIONS FOLLOWED FOR APPLICATION AND DRYING/CURING FOR THE TOUCH-UP PAINTING CONDUCTED ON THE POTABLE WATER STORAGE TANKS. THIS IS A NEW REQUIREMENT OF THE AUGUST 2005 VSP OPERATIONS MANUAL.
Recommendation: 5.3.1.1.3 Ensure that interior coatings on potable water tanks are approved for potable water contact, and all manufacturer"s recommendations for application, drying, or curing shall be followed. Ensure that documentation for the coating used and recommendations followed is available for review during inspections.

Violation: THERE WAS NO DOCUMENTATION THAT THE POTABLE WATER TANKS WERE FLUSHED TO 5 PPM OR LESS PRIOR TO PLACING THEM BACK INTO SERVICE FOLLOWING DISINFECTION.
Recommendation: Ensure that the documentation of inspection, maintenance, cleaning, disinfection (to include concentration and contact time of disinfectant), and flushing is maintained for 12 months and is available to the VSP for review during inspections.

Violation: THE LEFT WALL OF THE TECHNICAL SPACE UNDER THE ICE DISPENSER WAS SOILED WITH A BROWN SLIMY MATERIAL.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: PERSONS LEAVING THE PUBLIC TOILETS MUST TOUCH THE HANDLE WITH THEIR BARE HANDS TO EXIT THE TOILET ROOM. THIS IS A NEW REQUIREMENT OF THE AUGUST 2005 VSP MANUAL.
Recommendation: 9.1.1.1.3 Ensure that public toilet facilities are equipped so that persons exiting the toilet room are not required to handle the door with bare hands.

Violation: THE HOBART FLASH BAKE OVENS WERE POSTED NOT IN USE AND WERE AWAITING REMOVAL.

Violation: SLOTTED FASTENERS WERE NOTED ON THE FOOD CONTACT SURFACES OF THE CORNELIUS ICE MACHINES MAKING THE AREAS DIFFICULT TO CLEAN.
Recommendation: Replace the slotted fasteners with non-corroding, non-slotted fasteners.

Site: Bar-CEZANNE COFFEE BAR
Violation: LIDS TO DISPOSABLE COFFEE CUPS, PITCHERS, AND OTHER FOOD SERVICE ITEMS WERE STORED DIRECTLY NEXT TO THE HANDWASHING SINK, THE SINK WAS NOT EQUIPPED WITH SPLASH SHIELDS, SUBJECTING TO THE CLEAN ITEMS TO CONTAMINATION FROM THE HANDWASHING SINK.
Recommendation: Discontinue storing food service items near the handwashing sink or install a splash shield to protect the items.

Site: Food Service General-UPRIGHT REFRIGERATORS AND FREEZERS
Violation: THE TECHNICAL COMPARTMENTS ABOVE THE DOORS WERE SOILED WITH DUST AND DEBRIS. SOME OF THE DEBRIS APPEARS TO STILL BE FROM THE ORIGINAL CONSTRUCTION. SOME OF THESE UNITS HAD CONDENSATE RUNNING DOWN THE FRONT OF THE UNITS ONTO THE DOORS.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Provisions-MEAT FREEZER
Violation: THE DECK TILES ON THE RIGHT SIDE OF THE DIAMOND PLATE DECKING WERE CRACKED AND BROKEN MAKING CLEANING DIFFICULT.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Site: Provisions-DRY STORE ROOM
Violation: AN OPEN BAG OF POTATO CHIPS AND AN OPEN BAG OF COOKIES WERE FOUND ON TOP OF FOOD BOXES IN THE DRY STORAGE ROOM. ONLY CREW MEMBERS WERE WORKING IN THE PROVISION ROOMS.
Recommendation: Ensure that employees eat, drink, or use any form of tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils, and linens; unwrapped single-service and single-use articles; or other items needing protection can not result.

Site: Provisions-DAIRY AND FRUIT WALK-IN REFRIGERATORS
Violation: LIQUID WAS NOTED LEAKING FROM DECKHEAD SEAMS IN THESE WALK-IN REFRIGERATORS, IT WAS NOT OBSERVED DRIPPING ON ANY FOOD CONTAINERS. ADDITIONALLY, PINK SLIME WAS NOTED IN SOME OF THESE SEAMS.
Recommendation: Identify the source of the leaking liquid and correct it. Ensure that decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, are cleaned as often as necessary.

Site: Medical-SAMPLE COLLECTION AND SHIPPING MATERIALS
Violation: THE SAMPLE COLLECTION CONTAINERS WERE AVAILABLE IN MEDICAL BUT THE APPROVED SHIPPING CONTAINERS AND LABELS WERE NOT AVAILABLE. THIS IS A NEW REQUIREMENT OF THE AUGUST 2005 VSP OPERATIONS MANUAL.
Recommendation: Provide approved shipping containers and labels for shipping clinical samples.
