Site: Medical-MEDICAL
Violation: THE SHIP DID NOT HAVE THE REQUIRED 10 SPECIMEN COLLECTION AND TRANSPORT CONTAINERS FOR VIRAL AND BACTERIAL SPECIMENS. THIS IS A NEW REQUIREMENT IN THE VSP OPERATIONS MANUAL.
Recommendation: 4.3.1.1 The medical staff will be responsible for maintaining a supply of at least 10 clinical specimen collection containers for both viral and bacterial agents (10 for each), as well as the proper shipping containers and labels for same and provided in Annex 13.4 of this manual. Specific information on vendors where supplies may be ordered is provided in the VSP website. 4.3.2.2 All clinical specimens shall be packaged and shipped in accordance with the guidelines outlined in Annex 13.4 and the specific details provided in the VSP website. The specimen packaging shall include the proper documentation as required by the receiving laboratory.

Violation: THE SHIP DOES NOT HAVE THE APPROPRIATE OUTER SHIPPING CONTAINERS FOR TRANSPORTING DIAGNOSTIC SPECIMENS. THE SHIP DOES NOT HAVE THE NECESSARY SHIPPING LABELS FOR TRANSPORTING DIAGNOSTIC SPECIMENS UNDER CURRENT DOT AND IATA REGULATIONS. THIS IS A NEW REQUIREMENT UNDER THE VSP OPERATIONS MANUAL.
Recommendation: Obtain the required shipping containers for transporting diagnostic specimens. Obtain the proper shipping labels for transporting diagnostic specimens under DOT and IATA hazardous materials regulations.

Site: Galley-MAIN GALLEY - PASTRY (PORT)
Violation: ONE LARGE CAN (106 OZ) OF TROPICAL FRUIT SALAD WAS FOUND SEVERLY DENTED ON THE SIDE. TWO DENTS WERE FOUND ON THE TOP SEAM AND ONE DENT WAS FOUND ALONG THE BOTTOM SEAM. THIS ITEM WAS DISCARDED.
Recommendation: Ensure that food packages are in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Canned goods with dents on end or side seams may not be used.

Violation: TWO LARGE BAGS OF GOURMET POPCORN WERE STORED DIRECTLY ON THE DECK.
Recommendation: Ensure that food is protected from contamination by storing the food: (1) In a clean, dry location; (2) Where it is not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.

Recommendation: Ensure that exhaust ventilation hood systems in food preparation and warewashing areas including components such as hoods, fans, guards, and ducting are designed to prevent grease or condensation from draining or dripping onto food, equipment, utensils, linens, and single-service and single-use articles.
