Violation: ON THE CRUISE ENDING 02/18/2006, ONE CREWMEMBER WAS MISCLASSIFIED AS A NON-CASE. ON THE CRUISE ENDING 01/28/2006, ONE PASSENGER WAS MISCLASSIFIED AS A NON-CASE.
Recommendation: Ensure that all gastrointestinal illness cases are classified appropriately.

Violation: ON TWO SEPARATE CRUISES (02/18/06, 01/28/06) A CREW CASE AND AN PASSENGER CASE WERE NOT ACCURATELY REPORTED ON THE 24-HOUR OR 4-HOUR UPDATE REPORTS. THESE CASES WERE MISCLASSIFIED AS NON-CASES. IN ADDITION, ON THE CRUISE ENDING 02/25/06, ONE 4-HOUR REPORT WAS NOT SUBMITTED AS REQUIRED.
Recommendation: If the number of cases changes after submission of the initial report, ensure that an updated report is submitted no less than 4 hours before the vessel's arrival at the U.S. port.

Violation: A CHAIR WAS FOUND STORED ON THE TOP OF THE CLEAN DRINK PREPARATION COUNTERTOP.
Recommendation: Ensure that chairs are not place on top of preparation counters. Ensure that food is protected from contamination by storing the food: (1) In a clean, dry location; (2) Where it is not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.

Violation: ALL BUT ONE OF THE SPRAY NOZZLES OF THE UPPER FINAL RINSE SPRAY ARM WERE CLOGGED (FIRST SPRAY ARM). SEVERAL OF THE SPRAY NOZZLES IN THE SECOND SPRAY ARM DID NOT DISPLAY A FAN-LIKE SPRAY PATTERN. THE FINAL RINSE PRESSURE GAUGE WAS READING 0.3 BAR AND THE MACHINE WAS IN ACTIVE USE AT THE TIME OF THE INSPECTION. THIS WAS CORRECTED DURING THE INSPECTION.
Recommendation: Unclog the upper final rinse spray nozzles.

Violation: SEVERAL OF THE SERVING TRAYS WERE DAMAGED ALONG THE EDGES, MAKING CLEANING DIFFICULT.

Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: THE FORM USED TO ASK RETURNING OR NEW CREW MEMBERS ABOUT THEIR ILLNESS HISTORIES 3 DAYS PRIOR TO BOARDING, ONLY LISTS DIARRHEA AS A SYMPTOM. THE FORM SHOULD INCLUDE VOMITING TO MEET THE MINIMUM SYMPTOMS OF THE OPERATIONAL DEFINITION OF A GASTROINTESTINAL ILLNESS. THE CORPORATE OFFICE IS DEVELOPING A SYSTEM TO LOG ALL SYMPTOMS (STAR-DOCS) THAT WILL BE IN COMPLIANCE WITH THE REQUIREMENTS OF VSP OPERATIONS MANUAL - 2005.
Recommendation: Ensure that the questionnaire also includes all symptoms associated with gastrointestinal illness as per the VSP definition.

Violation: THE SHIP DOES NOT HAVE THE REQUIRED OUTER CONTAINERS AND REQUIRED SHIPPING LABELS TO TRANSPORT DIAGNOSTIC SPECIMENS UNDER THE IATA AND DOT SHIPPING STANDARDS. ACCORDING TO THE HEAD NURSE, THE CORPORATE OFFICE HAS INTENTIONS OF BRINGING THE CONTAINERS WITH THEM IN THE EVENT SHIPPING OF THESE SPECIMENS BECOMES NECESSARY. THIS IS A NEW REQUIREMENT UNDER THE VSP OPERATIONS MANUAL - 2005.
Recommendation: Ensure the medical staff maintains a supply of shipping containers and labels to transport diagnostic specimens.

Violation: THE WAREWASH AREA WAS VERY HOT AND HUMID. CONDENSATE WAS NOTED ON THE DECKHEAD THROUGHOUT THIS AREA.
Recommendation: Ensure that ventilation hood systems and devices are sufficient in number and capacity to prevent grease or condensation from collecting on bulkheads and deckheads.
