Site: Medical-MEDICAL - GI ILLNESS REPORTING
Violation: A REVIEW OF THE RECORDS OF GI ILLNESS REPORTS FOR THE LAST FIVE CRUISES, INDICATED THAT SEVERAL REPORTABLE CASES LISTED IN THE GI LOGS WERE NOT REPORTED ON THE 24-HOUR ROUTINE REPORT AND ON THE LAST VOYAGE, THE REPORTABLE CASES WERE NOT LISTED ON THE 4-HOUR UPDATE REPORT. THE CRUISES IN QUESTION HAD DISEMBARKATION DATES AS FOLLOWS: 01/29/2006 01/15/2006 01/07/2006 ON THE VOYAGE ENDING 01/29/2006, TWO PASSENGER CASES WERE NOT INCLUDED IN THE 24-HOUR OR 4-HOUR REPORTS. THE CASES WERE LISTED AS NOT REPORTABLE ON THE GI LOG, BUT MET THE OPERATIONS CASE DEFINITION. FOR THE VOTAGE ENDING 01/15/2006, THE 24-HOUR REPORT LISTED ONLY ONE (1) REPORTABLE CASE, BUT A REVIEW OF THE GI LOG LISTED A TOTAL OF 9 REPORTABLE CREW CASES THAT REPORTED TO THE INFIRMARY BEFORE THE SUBMISSION OF THE 24-HOUR REPORT. A TOTAL OF 12 CREW CASES WERE LISTED IN THE GI LOG FOR THE ENTIRE CRUISE AND THIS COUNT WAS REFLECTED IN THE 4-HOUR UPDATE REPORT. IN ADDITION, A TOTAL OF 8 PASSENGER CASES WERE LISTED IN THE 24-HOUR REPORT, BUT THE GI LOG LISTED ON 3 PASSENGER CASES. ACCORDING TO THE GI LOG, THE COUNT OF PASSENGER CASES SHOULD HAVE BEEN 3 VICE 8. ON THE VOYAGE ENDING 01/07/2006, THREE CREW CASES WERE NOT REPORTED ON 24-HOUR REPORT. IN EACH INSTANCE, THE CASE HAD BOTH VOMITING AND DIARRHEA. THE 4-HOUR UPDATE REPORT LISTED THE CORRECT COUNTS OF 4 CREW AND 7 PASSENGER CASES. IN NONE OF THE ABOVE CASE DISCREPANCIES WERE THERE ANY NOTATIONS CLARIFYING IF THERE WAS UNDERLYING CO-MORBIDITY/ UNDERLYING ILLNESS (E.G., IRRITABLE BOWEL SYNDROME) THAT COULD ACCOUNT FOR THE REPORTING DIFFERENCES.
Recommendation: The gastrointestinal illness report shall contain: (1) The name of the vessel; (2) The ports of embarkation and disembarkation; (3) The dates of embarkation and disembarkation; (4) The total numbers of reportable cases of gastrointestinal illness among passengers and crew members, including those who have been disembarked or removed because of illness, even if the number is 0; and (5) The total number of passengers and crew members on the cruise. If the number of cases changes after submission of the initial report, ensure that an updated report is submitted no less than 4 hours before the vessel's arrival at the U.S. port.

Violation: A REVIEW OF THE LAST FIVE GI LOGS INDICATED THAT CREW POSITION WAS NOT LISTED FOR MOST CREW CASES. IN ADDITION, SEVERAL PASSENGER AND CREW CASES THAT MET THE OPERATIONAL CASE DFINITION WERE MISCLASSIFIED AS NON-CASES.
Recommendation: A reportable case of gastrointestinal illness shall be defined as: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period); or (2) Vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever; and (3) Reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member. Include crew position on the Gastrointestinal Illness Surveillance Log.

Violation: THE VESSEL IS NOT USING THE UPDATED GI ILLNESS LOG FORMAT THAT INCLUDES THE ADDITIONAL SYMPTOMS OF HEADACHE, MYALGIA AND ABDOMINAL CRAMPS. THESE ADDITIONAL SYMPTOMS ARE A COMPONENT OF THE OPERATIONAL CASE DEFINITION FOR GI ILLNESS. THIS IS A NEW REQUIREMENT UNDER THE VSP OPERATIONS MANUAL - 2005.
Recommendation: Provide and use a gastrointestinal illness surveillance log which includes all the data elements provided in the VSP form provided in the 2005 Operations Manual section 13.2.2, page 134.

Violation: THE VESSEL DOES NOT HAVE THE REQUIRED SHIPPING CONTAINERS AND LABELS FOR THE PROPER TRANSPORTATION OF CLINICAL SPECIMENS UNDER THE REQUIREMENTS OF IATA AND DOT. THIS IS A NEW REQUIREMENT UNDER THE VSP OPERATIONS MANUAL - 2005.
Recommendation: 4.3.2.2 Provide shipping containers and labels as outlined in Annex 13.4.

Site: Potable Water-CROSS-CONNECTION CONTROL PROGRAM
Violation: THE WRITTEN LISTING OF BACKFLOW PREVENTION DEVICES AND METHODS WAS NEARLY COMPLETE, WITH BOTH DEVICES AND AIR-GAPS LISTED IN MOST AREAS. THE LIST DID NOT INCLUDE AIR-GAPS INSTALLED IN THE VARIOUS CHEMICAL DISPENSING BOTTLES ATTACHED TO BULKHEADS IN LOCATIONS THROUGHOUT THE SHIP, AIR-GAPS AT DRAINS FOR THE MINERALIZER AND WATER SOFTENER TANKS IN THE ENGINE SPACES, AND THE AIR-GAPS ON THE VARIOUS UPRIGHT ICE MACHINE POTABLE WATER FEED LINES THROUGHOUT THE SHIP.
Recommendation: 5.7.1.1.4 A cross-connection control program shall inclde at a minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so there is a match to the plumbing system component and location, the program shall set a schedule for inspection frequency. Repeat devices such as toilets can be grouped under a single device type, and air-gaps shall be included in the listing. A log documenting the inspection or maintenance in written or electronic form shall be maintained and be available for review during inspections.

Violation: ALTHOUGH THE WHIRLPOOL SPAS ARE BACKWASHED DAILY, THERE IS NO VISUAL CONFIRMATION THAT THE WATER IS CLEAN FLOWING, BECAUSE THE SYSTEM BACKWASH IS AUTOMATIC AND NONE OF THE STAFF MAKE AN OBSERVATION OF THAT BACKWASH CYCLE.
Recommendation: 6.3.1.1.4 Granular filters shall be backwashed at least daily. Backwashing shall be repeated until the water viewed through the sight glass or discharge point is clean flowing.

Violation: A PAN OF ROASTED BELL PEPPERS WERE FOUND WITH AN INTERNAL PRODUCT TEMPERATURE OF 47 F. THESE ITEMS WERE REMOVED AND PLACED INTO THE BLAST CHILLER FOR RAPID COOLING.
Recommendation: Ensure that potentially hazardous food is maintained: (1) At 60C (140F) [roasts 54C (130F)] or above, or (2) At 5C (41F) or less.

Violation: THERE WAS INSUFFICIENT SPACE AT THIS AREA FOR STAGING OF DIRTY POTS/PANS/UTENSILS. THE ENTIRE UNDERCOUNTER STORAGE SPACE AT THE DROP-OFF COUNTER WAS FULL, THE DROP-OFF COUNTER HAD ITEMS STACKED TO WITHIN 1/2 METER OF THE DECKHEAD, THE LARGE DIRTY RACK NEAR THE POTWASH MACHINE WAS FILLED TO CAPACITY, AND 4 DECKSTANDS SURROUNDING THE DIRTY RACK WERE STACKED AT LEAST A METER HIGH WITH SOILED ITEMS. THE DIRTY RACK FOR POTS IS POSITIONED AT THE CLEAN END OF THE OPERATION, AFTER THE SANITIZING SINK, DIRECTLY OPPOSITE THE POTWASH MACHINE, AND ALONG THE PATH TO THE CLEAN STORAGE RACKS. THIS IS OPPOSITE THE PROPER FLOW OF DIRTY TO CLEAN, AS CLEAN ITEMS MUST PASS THRU A DIRTY PATH TO REACH CLEAN RACKS. THERE WAS NEARLY A METER SPACE BETWEEN THE DIRTY STORAGE AND THE POTWASH MACHINE, BUT THE ARRANGEMENT SHOULD BE REFIGURED TO PREVENT CROSS-CONTAMINATION.
Recommendation: Ensure that drainboards, utensil racks, or tables large enough to accommodate all soiled and cleaned items that may accumulate during hours of operation are provided for necessary utensil holding before cleaning and after sanitizing. Some permanent redesign of this arrangement should be made at the next dry dock opportunity.

Violation: THE CONVEYOR ON THE DISHWASH UNIT IS MISSING PLATE GUIDES. THE OPERATOR IS LOADING (RACKING) PLATES INTO AREAS WHERE THE PLATE GUIDES ARE MISSING, RESULTING IN THE PLATES FALLING OVER, IMPEDING PROPER CLEANING AND SANITIZING.
Recommendation: Ensure that soiled items to be cleaned in a warewashing machine are loaded into racks, trays, or baskets or onto conveyors in a position that: (1) Exposes the items to the unobstructed spray from all cycles; and (2) Allows the items to drain. Ensure that warewashing equipment is maintained in good repair and proper adjustment including: (1) Being maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; and (2) Maintaining water pressure, and water temperature measuring devices in good repair and ensuring their accuracy within the intended range of use.

IT WAS UNCLEAR IF THESE BINS WERE TO BE USED FOR SILVERWARE DISPLAY, BUT ALL WERE IMMEDIATELY REMOVED TO THE DISHWASH AREA.
Recommendation: Ensure that cleaned equipment and utensils, laundered linens, and single-service and single-use articles are stored: (1) In a clean, dry location; (2) Where they are not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.

Violation: FOUR PREVIOUSLY CLEANED PITCHERS FOR COFFEE CREAMER AND MILK WERE SET WET AND INVERTED ON A SOLID SERVING TRAY ON THE CLEAN STORAGE RACK. THE CREAM LIDS WERE STORED BESIDE WITH THE FOOD CONTACT PORTION ON THE TRAY SURFACE. AS THOSE ITEMS DRAINED WATER IT POOLED ON THE TRAY AND THE CREAMER LIDS WERE STANDING IN POOLED WATER.
Recommendation: Ensure that cleaned equipment and utensils, laundered linens, and single-service and single-use articles are stored: (1) In a clean, dry location; (2) Where they are not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.

THE WASTE RECEPTACLES IN EACH WERE A MECHANICAL TYPE WITH A FOOT PEDAL CONTROLLING OPENING OF THE LID. IN BOTH TOILET ROOMS THE LID FOR THE WASTE RECEPTACLE DID NOT FUNCTION FROM THE FOOT PEDAL, REQUIRING USERS TO HANDLE THE LID TO PHYSICALLY OPEN IT. BOTH TOILETS HAD AN OPEN BOX OF FACIAL TISSUE AT THE SINK BASIN AND A REPLACEMENT BOX STORED ABOVE WITH REPLACEMENT TOILET TISSUE ROLLS.
Recommendation: Ensure that handwashing facilities are kept clean and in good repair. Ensure that each handwashing facility has a supply of hand-cleansing soap or detergent and a supply of single-service paper towels available.

Violation: THE CRUSHED ICE AND WATER DISPENSER AREA WAS OPENED AND SOME SMALL PARTICLES WERE IDENTIFIED IN THE REAR ICE SECTION. AS THE FRONT PANEL WAS OPENED, 4 SMALL FRUIT OR DRAIN FLIES APPEARED NEAR THE DISPENSING SECTION AND THE DRAIN FROM THE UPPER, FRONT LEFT COMPARTMENT. THIS PEST SIGHTING WAS REPORTED TO THE PEST MANAGEMENT CONTROL OFFICER ONBOARD.
