Violation: DUST WAS NOTED ON THE DECK IN THE LOCKERS AT THE AFT SECTION OF THE GALLEY.
Recommendation: Ensure that decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, are cleaned as often as necessary.

Recommendation: Ensure that backflow preventers are maintained in good repair.

Violation: THE WRITTEN TIME CONTROL PLAN DID NOT DESCRIBE THE 4-HR DISCARD TIME LABELING OF POTENTIALLY HAZARDOUS FOODS. THE OPERATING TIME FOR THE CAFE WAS GREATER THAN 4 HOURS.
Recommendation: If time only, rather than time in conjunction with temperature, is used as the public health control for a working supply of potentially hazardous food before cooking, or for ready-to-eat potentially hazardous food that is displayed or held for service for immediate consumption, ensure that (1) The food is marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control; (2) The food is cooked and served, served if ready-to-eat, or discarded, within 4 hours from the time when the food is removed from temperature control; and (3) The food in unmarked containers or packages or marked to exceed a 4 hour limit is discarded. Ensure that written procedures to ensure compliance with these guidelines is maintained on the vessel and made available to the VSP, upon request.

Violation: THERE WERE NO PAPER TOWELS IN THE TOILET ROOMS. THIS REQUIRED INDIVIDUALS TO TOUCH THE DOOR HANDLES WITH BARE HANDS TO EXIT.
Recommendation: Equip public toilet facilities so that persons exiting the toilet room are not required to handle the door with bare hands. This may be accomplished by methods such as locating paper towel dispensers at sinks and waste containers near the room door, installing mechanically operated doors, door removal, or other effective means.

Violation: THE WATER TEMPERATURE AT THE HANDWASH SINK CLOSEST TO THE DISHWASH UNIT MEASURED 128F. THE SINK HAD AN ELECTRONIC SENSOR AND THE TEMPERATURE COULD NOT BE ADJUSTED BY THE USER.
Recommendation: Ensure that handwashing sinks are equipped to provide water at a temperature of at least 43C (110F) through a mixing valve or combination faucet. For handwash sinks with electronic sensors, where the user cannot make temperature adjustments, ensure that the temperature provided to the user does not exceed 52C (125F).

Recommendation: Place the ramekins on a wash rack to ensure they are properly washed and sanitized by the dishwash unit.

Violation: CLOSE CONTACTS OF ILL CREW MEMBERS WERE INTERVIEWED INITIALLY AND 24-HOURS AFTER ONSET OF ILL CREW MEMBER'S SYMPTOMS. THERE WAS NO INTERVIEW AT 48-HOURS AFTER ONSET.
Recommendation: Ensure that medical or supervisory staff conducts a verbal interview with asymptomatic cabin mates and immediate contacts of ill crew members to confirm their condition, advise them of the hygiene and handwashing facts, and to instruct them to report immediately to medical if they develop illness symptoms. Ensure verbal interviews are conducted daily until 48 hours after onset of the ill crew members symptoms began.

Site: Potable Water-POTABLE WATER - PRODUCTION CHLORINATION
Violation: THERE WERE 5 DAYS SINCE 22 MAY 2006 WHERE FOR 1 OF THE HOURLY CHLORINE MEASURES DURING POTABLE WATER PRODUCTION, THE FREE CHLORINE RESIDUAL WAS BELOW 2 PPM. IN ALL BUT ONE OF THESE INSTANCES, THERE WAS NO NOTATION ON THE MANUAL RECORD OF ANY ADJUSTMENT MADE TO INCREASE THE RESIDUAL, AND IN NONE OF THE CASES WAS THERE ANY RECORDED FOLLOW-UP TEST UNTIL THE FOLLOWING HOURLY CHECK TO INDICATE A LEVEL OVER 2 PPM WAS ACHIEVED.
Recommendation: Ensure that potable water is continuously halogenated to at least 2.0 mg/L (ppm) free residual halogen at the time of bunkering or production with an automatic halogenation device that is controlled by a flow meter.

Site: Recreational Water Facilities-POOLS - SOLARIUM, WAVE RIDER
Violation: THE COMBINATION OF WATER VOLUME, TEMPERATURE (29 C IN SOLARIUM AND 27 C IN WAVE RIDER), AND WATER FEATURES IN BOTH THE SOLARIUM AND WAVE RIDER POOLS MEETS THE VSP DEFINITION OF SPA POOL. THE HALOGEN RESIDUALS RECORDED SINCE THE SHIP BEGAN SAILING ON 22 MAY 2006 INDICATED LEVELS OF FREE CHLORINE BELOW 3 PPM FOR BOTH POOLS. DURING THE INSPECTION THE WAVE RIDER POOL HAD A 2.3 PPM RESIDUAL. THIS IS A NEW REQUIREMENT FROM THE 2005 VSP OPERATIONS MANUAL.
Recommendation: Ensure that spa pool halogen is operated and maintained according to whirlpool spa standards provided in sections 6.3.1.1.1 - 6.4.1.3.1, except that daily water change in section 6.3.1.2 is not required. If the spa pool uses only seawater the daily shock in section 6.3.2.1.2 is also not required.

Violation: THE POOL MAINTENANCE TANK INSTALLED DURING CONSTRUCTION OF THIS SHIP WAS DEEMED BY THE VSP TO BE AN IMPROPERLY CONSTRUCTED TANK, DUE TO THE WALLS SHARED WITH THE SHIP'S HULL AND WITH A WASTEWATER TANK. ADDITIONALLY, THE TANK INTERIOR WAS NOT COATED AS A POTABLE WATER TANK. THE RECIRCULATION FLANGE IN THE ENGINE ROOM IS NOW BLANKED CLOSED.
Recommendation: Ensure this tank is not used to re-supply any of the pools or whirlpool spas with water.
