Site: Galley-POTWASH AREA
Violation: SEVERAL BAIN MARIE PANS WERE WET STACKED IN THE CLEAN POTWASH STORAGE AREA. THIS AREA IS SCHEDULED FOR A MAJOR RENOVATION DURING THE NEXT FEBRUARY 2007 DRY DOCK. THE CURRENT CLEAN STORAGE SPACE IN THE POTWASH AREA WAS NOT ADEQUATE FOR THE NUMBER OF GALLEY WARE.
Recommendation: After cleaning and sanitizing, ensure that equipment and utensils are air-dried or adequately drained before contact with food. Ensure that symptomatic passengers meeting the defintion for AGE are advised to remain isolated in cabin until well for a minimum of 24 hours after symptom resolution.

Recommendation: Ensure that handwashing facilities are used for no other purpose and are accessible at all times.

Violation: THE INTERIORS OF THE UNDERCOUNTER CABINETS AND TECHNICAL SPACES THROUGHOUT THE PERMANENT BUFFET ISLANDS WERE UNFINISHED AND HAD DIFFICULT TO CLEAN GAPS, VOIDS, LEDGES AND EXPOSED UTILITY CONNECTIONS.
Recommendation: Ensure that equipment that is fixed because it is not easily movable is installed so that it is: (1) Spaced to allow access for cleaning along the sides, behind, under and above the equipment; (2) Spaced from adjoining equipment, bulkhead, and deckhead at a distance of not more than 0.8 millimeter or 1/32 inch; or (3) Sealed to adjoining equipment or bulkhead, if the equipment is exposed to spillage or seepage.

Violation: THE MEDICAL STAFF WAS NOT PROVIDING PASSENGERS DIAGNOSED WITH GASTROINTESTINAL ILLNESS A PERSONAL HYGIENE AND HANDWASH INFORMATION SHEET. THIS IS A NEW REQUIREMENT UNDER THE VSP OPERATIONS MANUAL - 2005.
Recommendation: Ensure that symptomatic passengers meeting the defintion for AGE are provided handwashing tips and personal hygiene information.

Site: Medical-CASE DEFINITION
Violation: ON 29 JUNE 2006 (VOYAGE 15457) A CREW CASE (47 Y/O MALE ENTERTAINER) WAS LISTED AS A NON-CASE AND THE UNDERLYING ILLNESS WAS LISTED AS "INTOLERANCE" ACCORDING TO THE GI LOG, THE CASE HAD EXPERIENCED 3 EPISODES OF DIARRHEA AND 3 EPISODES OF VOMITING WITHIN A 24 HOUR PERIOD. THIS CASE WAS DISCUSSED WITH THE DOCTOR AND IT WAS NOT MADE CLEAR WHY THIS INDIVIDUAL WAS A NON-REPORTABLE CASE.
Recommendation: A reportable case of gastrointestinal illness shall be defined as: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period); or (2) Vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever (temperature >38 C (100.4 F)); and (3) Reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member.

Violation: FOUR KNIVES, TWO POTATO PEELERS AND A SHARPENING STEEL WERE FOUND STORED INSIDE A CLEANING LOCKER. THE CLEANING LOCKER WAS CLEARLY LABELED.
Recommendation: Ensure that cleaned equipment and utensils, laundered linens, and single-service and single-use articles are stored: (1) In a clean, dry location; (2) Where they are not exposed to splash, dust, or other contamination; and (3) At least 150 mm (6 inches) above the deck.

Recommendation: Ensure that bulkhead and deck junctures are coved (including galleys, pantries, deck/counter junctures at buffets, bars, waiter stations, and dining room work counters).

Recommendation: Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance. Ensure that nonfood-contact surfaces of equipment and utensils are designed and constructed to be durable and to retain their characteristic qualities under normal use conditions.

Recommendation: Replace the dough sheeter belts.

Violation: A CAN OF FOOD WAS FOUND DENTED ALONG THE TOP SEAM. THIS ITEM WAS DISPOSED OF DURING THE INSPECTION.
Recommendation: Ensure that food packages are in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Ensure that canned goods with dents on end or side seams are not used.

Violation: DURING MANUAL POTWASH OPERATIONS, PERSONNEL WERE OBSERVED DIPPING THE ITEMS IN THE FINAL SANITIZER SOLUTION FOR 1-2 SECONDS. THE SANITIZER USED IN THE AREA WAS CHLORINE.
Recommendation: Ensure that the chemical sanitizer is used in accordance with the EPA-approved manufacturer's label use instructions at a minimum temperature of 24C (75F) with an exposure time of 7 seconds for a chlorine solution and 30 seconds for other chemical sanitizers.

Violation: THE DATA PLATE ON THE GLASSWASH MACHINE DID NOT LIST THE CYCLE TIMES.
Recommendation: Ensure that warewashing machines are provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Violation: THE SODA DISPENSER WAS LABELED "NOT IN USE" WHEN THE STAFF WAS ASKED THE DISPOSITION OF THE EQUIPMENT, IT WAS STATED THAT IT WAS NO LONGER NEEDED AND WAS DUE TO BE REMOVED AT THE NEXT DRYDOCK PERIOD IN FEBRUARY 2007.
Recommendation: Remove this piece of equipment if there are no future plans for its use.

Violation: A BAR TENDER WAS OBSERVED WASHING HIS HANDS IN A DUMP SINK WITHOUT SOAP AND FOR SIGNIFICANTLY LESS THAN THE REQUIRED 20 SECONDS. A PROPER HANDSINK WAS AVAILABLE IN THIS AREA.
Recommendation: Ensure that food employees clean their hands and exposed portions of their arms with a cleaning compound in a handwashing sink by vigorously rubbing together the surfaces of their lathered hands and arms for at least 20 seconds and thoroughly rinsing with clean water and pay particular attention to the areas underneath the fingernails and between the fingers.

Violation: A SANITIZER CONCENTRATION IN THE SANITIZER BUCKET LOCATED IN THE BAR SERVICE AREA WAS TESTED AT LESS THAN 50 PPM. THIS ITEM WAS CORRECTED DURING THE INSPECTION.
Recommendation: Ensure that a chlorine solution used as a sanitizing solution has a concentration between 50 mg/L (ppm) and 200 mg/L (ppm).

Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.
