Site: Medical-MEDICAL - GI LOG
Violation: DURING VOYAGE 20060617007, A CREW CASE WAS LISTED AS A NON-CASE IN THE NEW STARDOCS SYSTEM. ACCORDING TO THE ENTRY IN STARDOCS, THE CASE HAD 2 EPISODES OF DIARRHEA AND ONE EPISODE OF VOMITING WITHOUT ANY NOTATION OF UNDERLYING MEDICAL CONDITION. THE CASE DEFINITION IN THE VSP OPERATIONS MANUAL INCLUDES VOMITING AND ONE ADDITIONAL SYMPTOM, WHICH INCLUDES A SINGLE EPISODE OF DIARRHEA WITH VOMITING. IT APPEARS THAT THE STARTDOCS SYSTEM DID NOT DEFINE THIS PERSON AS A CASE OR THAT THE PERSON ENTERING THE DATA OVERRODE STARDOCS.
Recommendation: Evaluate the case definition in STARDOCS to ensure that it captures cases as reportable when vomiting is present with diarrhea. If the reasons for the misclassification is due to overriding the STARDOCS system, instruct data entry personnel in the proper use of this feature.

Site: Medical-MEDICAL - DISEASE REPORTING
Violation: DURING VOYAGE 20060617007, A CREW CASE WAS NOT REPORTED ON EITHER THE 24 HOUR OR 4 HOUR REPORTS AS A CASE DUE TO THE STARDOCS SYSTEM MISCLASSIFYING THE CASE AS A NON-CASE. THE CASE EXPERIENCED TWO EPISODES OF DIARRHEA AND ONE EPISODE OF VOMITING.
Recommendation: Ensure all case-patients are properly coded as a case or non-case based on the current VSP operational definition and ensure that all cases are reported in the 24 and 4-hour reports.

Violation: THE HANDWASH STATION IN THIS AREA WAS PARTIALLY BLOCKED BY A PLATE TROLLEY. THE AREA WAS ACTIVELY BEING USED AT THE TIME OF THE INSPECTION. THIS ITEM WAS CORRECTED DURING THE INSPECTION.
Recommendation: Ensure the handwash station is completely accessible when the area is operational.

Violation: A TELEPHONE WAS FOUND STORED INSIDE THE CONDIMENT STORAGE CABINET OF THE WMF COFFEE MAKER. THIS ITEM WAS CORRECTED DURING THE INSPECTION.
Recommendation: Ensure that only articles necessary for the food service operation are stored in food preparation, food storage, and warewashing areas.

Violation: A SHEET CAKE WAS FOUND INSIDE THE WALK-IN REFRIGERATOR UNCOVERED AND STORED ON THE TOP SHELF OF THE STORAGE RACK. A HAIR WAS FOUND ON TOP OF THE CAKE. THIS ITEM WAS DISCARDED DURING THE INSPECTION.
Recommendation: Ensure that food is protected from contamination by storing the food: (1) In a clean, dry location; (2) Where it is not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.

Violation: A WET WIPING CLOTH WAS FOUND DRAPED ACROSS A SANITIZER BUCKET.
Recommendation: Ensure that wiping cloths used for wiping food spills are dry and used for wiping food spills from tableware and single-service articles; or wet and cleaned, stored in a chemical sanitizer, and used for wiping spills from food-contact and nonfood-contact surfaces of equipment.

Violation: A OPEN CUP OF AN ORANGE-COLORED DRINK WITH A DRINKING STRAW AND NO LID WAS FOUND STORED ON TOP OF THE HANDWASH STATION SHELF BEHIND THE BAR SERVICE COUNTER. IT DID NOT APPEAR THAT THIS CONTAINER WAS PLACED THERE BY A PASSENGER, SINCE THE LOCATION IN WHICH THE ITEM WAS STORED WAS DIFFICULT TO ACCESS FROM THE PASSENGER SIDE OF THE COUNTER.
Recommendation: Ensure that employees eat, drink, or use any form of tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils, and linens; unwrapped single-service and single-use articles; or other items needing protection can not result.
