Site: Children Area-ADVENTURE ZONE
Violation: THE VINYL COVERING ON THE FLOOR PILLOWS WAS TORN, EXPOSING THE OPEN CELL PADDING THAT WAS NOT CLEANABLE.
Recommendation: Ensure that surfaces of tables, chairs, and other furnishings that children touch with their hands are cleanable.

Site: Potable Water-CROSS-CONNECTION CONTROL PROGRAM
Violation: THE ENGINEER'S LISTING FOR BACKFLOW PREVENTION DEVICES DID NOT LIST AIR GAPS AS PART OF THEIR CROSS-CONNECTION CONTROL PROGRAM.
Recommendation: Ensure that air gaps are listed as part of the cross-connection control program.

Site: Potable Water-POTABLE WATER HOSES
Violation: THE POTABLE WATER HOSES WERE NOT MARKED "POTABLE WATER ONLY." THIS IS A NEW 2005 VSP REQUIREMENT.
Recommendation: Ensure that potable water hoses are labeled for use with the words "potable water only" in letters at least 13mm (0.5 inch) at each connecting end.

Site: Buffet-SERVICE LINES
Violation: A FEW OF THE DOORS FOR THE UNDERCOUNTER HOT HOLDING UNITS WERE NOT PROPERLY ADJUSTED. SOME OF THE DOOR GASKETS WERE NOT PROPERLY INSTALLED.
Recommendation: Ensure that non-food contact equipment is maintained in good repair and proper adjustment including: (1) Equipment maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines. (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates are kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Site: Buffet-BEVERAGE SERVICE LINES
Violation: THE NEAREST HANDWASHING STATION FOR THE BEVERAGE COUNTERS WAS GREATER THAN 8 METERS (26 FEET) AWAY. THE INSTALLATION OF THIS HANDWASH STATION WILL BE A MAJOR RENOVATION AND IS SCHEDULED FOR THE NEXT DRY DOCK IN 2007. THESE SERVICE LINES WERE BEING OPERATED BY STAFF.
Recommendation: Ensure that the handwashing facility is located within 8 m (25 feet) of all parts of the area and is not located in an adjacent area that requires passage through a closed door. where the user makes hand contact with the door.

Site: Medical-GI LOG
Violation: ON TWO OF THE LAST FIVE CRUISES, ONE PERSON (TWO TOTAL) WAS DISPENSED ANTI-DIARRHEAL MEDICATION, BUT WAS NOT LISTED ON THE GI LOG. THE INDIVIDUALS DID NOT MEET THE CASE DEFINITION.
Recommendation: The log shall list: (1) All reportable cases of gastrointestinal illness; (2) All passengers and crew members who are dispensed antidiarrheal medication from the master of the vessel, the medical staff, or other designated staff.

Violation: A WRITTEN PERSONAL HYGIENE AND HANDWASHING TIP SHEET WAS NOT PROVIDED TO PASSENGER CASES OF ACUTE GASTROENTERITIS.
Recommendation: Ensure that symptomatic passengers meeting the defintion for AGE are provided handwashing tips and personal hygiene information.

Site: Other-OFFICER/STAFF MESS - DISHWASH AREA
Violation: THE PRESSURE GAUGE ON THE CONVEYOR-TYPE DISHWASHER WAS NOT WORKING. TWO SPRAY NOZZLES ON THE UPPER FINAL RINSE SPRAY ARM HAD AN ERRATIC SPRAY PATTERN. THESE TWO ITEMS MAY BE RELATED. THE FINAL RINSE TEMPERATURE WAS MEASURED ABOVE 160F AT THE PLATE SURFACE.
Recommendation: Ensure that the pressure gauge is repaired or replaced. Ensure that all components of the dishwasher are operational prior to placing the machine into service.

Site: Galley-DECK 4 - POTWASH
Violation: DURING MANUAL POTWASH OPERATIONS THE POTWASH STAFF WAS OBSERVED PLACING PANS IN THE FINAL SANITIZING RINSE FOR LESS THAN SEVEN SECONDS. THE SANITIZER CONCENTRATION WAS BETWEEN 50 - 200 PPM.
Recommendation: Ensure that the chemical sanitizer is used in accordance with the EPA-approved manufacturer's label use instructions at a minimum temperature of 24C (75F) with an exposure time of 7 seconds for a chlorine solution and 30 seconds for other chemical sanitizers.

Site: Galley-DECK 3 - SAUCE STATION
Violation: THE HANDWASH STATION IN THIS LOCATION WAS MISSING A "WASH HANDS OFTEN" SIGN. THIS ITEM WAS CORRECTED IMMEDIATELY.
Recommendation: Ensure that a sign stating "WASH HANDS OFTEN" in a language that the food employees understand is posted over handwashing sinks.

Site: Galley-DECK 2 - POTWASH
Violation: DURING MANUAL POTWASH OPERATIONS, STAFF WAS OBSERVED PLACING ITEMS IN THE FINAL SANITIZER RINSE FOR LESS THAN SEVEN SECONDS. THE STAFF WAS HEARD COUNTING FROM ONE TO SEVEN VERY QUICKLY. THE SANITIZER SOLUTION MEASURED BETWEEN 50-200 PPM.
Recommendation: Ensure that the chemical sanitizer is used in accordance with the EPA-approved manufacturer's label use instructions at a minimum temperature of 24C (75F) with an exposure time of 7 seconds for a chlorine solution and 30 seconds for other chemical sanitizers.

Site: Provisions-FROZEN MIXED WALK-IN REFRIGERATOR
Violation: A CAP AND GLOVES WERE NOTED STORED ON TOP OF FOOD ITEMS. THE FOOD ITEMS WERE IN SEALED CONTAINERS.
Recommendation: Ensure that food is protected from contamination by storing the food: (1) In a clean, dry location; (2) Where it is not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.
