This patient was photographed in Guatemala, Central America, exhibiting a condition known as typhus ulcerations of lower back and right hip, which was determined to be due to a case of typhus fever, also referred to as epidemic louse-borne typhus, caused by the bacterium, Rickettsia prowazekii.
