This photograph depicts a right lateral view of a premature newborn infant, weighing 4 lb 3 oz at birth, with a nontoxic diffuse goiter, evident as anterior cervical swelling. At birth, the infant required an exchange transfusion due to ABO incompatibility; the mother’s blood type was O, and the infant’s type was either A or B, leading to maternal antibody-mediated hemolysis of the infant’s red blood cells. The goiter was attributed to maternal ingestion of iodide-containing medications during pregnancy for asthma management. The infant was reported to have done well in the long term.
