This image shows cutaneous lesions consistent with erythema necroticans occurring in diffuse lepromatous leprosy associated with infection by Mycobacterium leprae. The condition involves vascular invasion of the skin, leading to erythema that may progress to necrotizing ulcerations. It is typically seen in untreated or relapsing diffuse lepromatous leprosy, including pure primitive diffuse lepromatous leprosy (PPDL). The lesions may appear as irregular or “bizarre” scars, some of which may be artifactual.
