This was a dorsal view of a patient’s swollen left foot, atop which a number of cutaneous lesions could be seen, which had been brought on by an infection known as actinomycosis, or mycetoma, caused by the Gram-positive, fungus like aerobic bacterium, Actinomyces israelii. Mycetoma is a slowly progressive, destructive infection of the cutaneous and subcutaneous tissues, and fascia, which can progress to a point where it can affect bone as well.
