This image shows a posterior view of a 3-day-old premature infant, demonstrating unilateral erythema of the back consistent with harlequin color change, a transient condition lasting only a few minutes. The left side of the body, up to the midline, shows deep red discoloration, associated with polycythemia and a temporary vasomotor instability commonly seen in preterm infants. This finding is considered nonpathologic and typically resolves by the third week of life.
